Receiving your REBATE is quick and easy.
To receive your rebate, you'll need to submit a copy of your original itemized receipt from Turkle & Associates showing proof of treatment, plus the sticker(s) from the EVOLENCE® box(es) that was used for your treatment. You'll also need to register on this website, https://www.evolence.com/us/rebate-register-us.jsp, then complete and submit an EVOLENCE® Rebate Request Form.
